Rhinos, Predators, and Landraiders all have two autolaunchers modeled on them. When triggering them, do you resolve 3 grenades from each launcher? If not, how do you determine where the grenades originate from?

The rules, as far as I read them, state that the 1st grenade is placed 6" directly in front of the tank, and the other two scatter left and right D3" from the centre of it.
Thats how ive been using it, and it keeps it simple and with no room to mess about with it, you want to hit somthing with them, position the tank right.
It might be possible that you can choose a point 6" away within the front 45 degree arc to scatter as above, but personally I think auto launchers are fixed on vehicles, and as such would fire in a fixed pattern.
Oh, when you fire, its only 3 grenades!!! No more. and you can only fire them twice, or chaos ones once.

Yep, just as James says with a fixed placement in front for the first grenade.
